Reducing Health Inequalities

We are increasingly providing hospital treatments as day cases - reducing the time people need to spend in hospital - and services such as specialist nursing, that were once only provided in hospital, can now be provided in local communities and in people’s homes. The exciting potential of technology has accelerated the opportunity to offer convenient care and support closer to home for people. Doctors, nurses and therapists can now access the information they need anywhere, anytime to support patients. Joined up hospital and community health and care services will enable a better experience and recovery for people that for example have cancer, have a stroke, or have respiratory disease.

By working together in these more integrated ways we will be able to achieve improved health outcomes and reduce inequalities.
